You've been invited by your GP practice to complete your NHS Health Check - a free, routine assessment for adults aged 40-74 across the country. This is part of a national programme that helps spot early signs of conditions such as heart disease, stroke, type 2 diabetes, kidney disease and some types of dementia.

The NHS Health Check is offered every five years to people who are not already known to have these conditions. It's an important part of staying well and preventing future illness.

This helps your healthcare team prepare for your appointment and tailor the advice they give you - making your face-to-face visit quicker, more focused, and more valuable. .

What happens next?

  1. Start by filling out a short online form using the secure Access Portal below. You'll be asked a few basic questions to register your interest and share some background health information. Please complete this within the next 7 days.
  2. Book your appointment. After submitting the form, you'll be contacted to schedule your NHS Health Check appointment.
  3. Attend your appointment. A healthcare professional will check your height, weight, blood pressure, and may take a blood sample to assess your cholesterol and blood sugar levels. They'll also ask about your medical and family history, and offer advice based on your individual risk factors.
