Blood Pressure Support Programme

Your surgery is participating in a local enhanced patient support programme, in partnership with Cambridgeshire and Peterborough ICB, to help support the care of people who may be at an increased risk from high blood pressure.

You have been identified as someone who may benefit from a review of your blood pressure.

Why Is This Important?

High blood pressure, or hypertension, is when the force of your blood pushing against your blood vessels is too strong. This can damage your blood vessels over time and lead to health problems like heart disease or strokes. This is usually recognised as a reading above 135/85mmHg at home, or 140/90mmHg in clinic.

A high blood pressure usually doesn't cause any symptoms (meaning you probably won't feel anything unusual), so having your blood pressure checked regularly allows you and your healthcare team to keep an eye on the condition and make lifestyle or medication changes to control your blood pressure and reduce your health risk.

This service aims to help you organise an up-to-date blood pressure being taken, which can be done at your community pharmacy or at home. If appropriate, suggestions for improved management of your blood pressure may be offered.

Please Complete Your Questionnaire

Simply access your secure portal below using your NHS number (provided in the text you received) and your date of birth. This will take you to a short questionnaire about arranging a blood pressure check and the opportunity to provide some more information about lifestyle factors which may impact your health. This is a secure NHS portal. All information entered is confidential and will only be shared with your participating GP practice.

If you are unable or would prefer not to have this check, please still fill out the form to enable us to record your personal preferences.
