Heart Health Support Programme

Your GP surgery is taking part in a local NHS programme to help people keep their hearts healthy. This programme is designed to lower the risk of heart disease and stroke by making sure your blood pressure is checked and, if needed, well managed.

You have been invited because your last recorded blood pressure was higher than usual. Even if you are already on medication for your blood pressure, this programme offers extra support to help you stay healthier.

Why is this important?

High blood pressure (hypertension) is when the force of your blood against the inside of your blood vessels is too strong. Over time this can damage your arteries and increase the risk of serious health problems like heart attacks and strokes.

You usually won't notice any symptoms, which is why checking your blood pressure regularly is so important. Keeping your readings in a healthy range reduces your risk and helps you feel better for longer.

How does the programme work?

If you join this free NHS service, we'll help you get an up-to-date blood pressure reading and, if it remains high, give you the right support to bring it down.

There are lots of ways to have your blood pressure checked:

  • At a participating community pharmacy
  • At your 40+ NHS Health Check (if invited)
  • Using a blood pressure monitor you own or have access to (please see the British Heart Foundation website for approved machines by following this link)
  • With a GP Surgery Pod at your GP surgery reception (where available)
  • Through the Essex County Council BP monitor Library Loan scheme (Essex only)

If needed, you may also be offered extra support such as:

  • Medical advice and medication reviews
  • Stop smoking services
  • Help with weight management
  • Support to be more physically active

What happens next?

Simply click the secure NHS portal link below and log in with your NHS number (provided in the text message you received) and your date of birth. From here, you'll be asked to complete a short questionnaire. This will let you:

  • Decide whether or not you'd like to take part
  • Tell us about your preferences for the programme
  • Share a little more about your lifestyle and health
  • Find out about next steps

Even if you choose not to join, please still complete the questionnaire so your preferences are recorded.
